Service Retirement

For active members who want to retire, please print and complete the forms below and read the information provided. Bring the completed forms to the Pension System office. An appointment with a Retirement Counselor is required for retirement. Please contact our office at 214-638-3863 to schedule an appointment.

Some of these forms may also be useful for pensioners.

• Pension Benefit Application
• Beneficiary Designee Selection (for final benefit payment)
• Member Information Consent form (The law protects your confidentiality as a Member of the Pension System. This form grants permission for the Pension System to share your name and photo in our communications (newsletters, Web, etc.).
• Authorization Agreement for Deductions (This form is OPTIONAL for the deductions listed on the form. If none of these are elected, the form is not required.)
• Direct Deposit Authorization form (This form is used to begin direct deposit or make changes to your direct deposit information.)
• IRS Form W-4P Federal Income Tax Withholding form

Additional Forms

• Change of Address form for Pensioners (Active Members must change their address with their department and through the City of Dallas Lawson System)
