2012 Open Enrollment for City of Dallas Insurance has been extended to Nov. 11, 2011

We received word from the City of Dallas that they are extending the deadline for open enrollment for Insurance until Friday, November 11. Here is the message:

Some of the retiree Enrollment Guides were delivered late. We apologize for the delay.

To ensure retirees have enough time to complete any changes to benefit elections, we have extended open enrollment for the retirees for two weeks - until Friday, November 11, 2011.

If you would like to make changes to your benefits, please call the Benefits Service Center at 888-752-9122.

Also, the City has notified us that there are also some misprinted phone numbers in the enrollment guide.
The retiree guide has two incorrect phone numbers on the following pages:

• Page 23: Post-65 Retiree section, the number is listed as:
  800-449-4954 and should be 800-392-7537 (it’s correct on page 49).
• Page 49: Vendor Contact List, the Medicare Part D Prescription Plan is listed as:
  Membership services (To ask questions only) 800-392-7527 and should be 888-556-6648
  City of Dallas Benefits Service Center (To enroll or change plans) of course should be 888-752-9122 (it’s correct on page 23)
